| Choice Health Network’s Upper Cumberland office is now located at 225 N. Willow Avenue, Suite D, in Cookeville. It currently provides affordable housing case management, including Housing Opportunities for Persons With AIDS (HOPWA), primary care and infectious disease care via telehealth, mental health counseling, emergency food and transportation aid, and naloxone distribution. Our goal is to increase access to care, remove barriers, and eliminate stigma. By offering a wider variety of services in rural areas, our clients are able to obtain care in a private, local setting, instead of having to drive to a bigger city for the same services. |
