POSITION SUMMARY:

The Ryan White Case Manager provides comprehensive case management services including patient advocacy, care coordination, and navigation of medical and social support services to People Living with HIV/AIDS (PLWH).

Work involves providing supportive services to promote participant access to medical care and increase participant self-sufficiency; performing administrative tasks involved in the review and maintenance of a caseload of program participants. The principal duties are performed in a clinical office environment where participants receive medical care and case management.

RESPONSIBILITIES AND DUTIES:

  • Conduct Ryan White case management: Provide intakes, assessments, and ongoing case management for a caseload of participants in the Ryan White program.
  • Assess and address barriers: Identify participant strengths and barriers to stability; aid in reducing barriers by connecting participants to resources and services.
  • Develop individualized care plans: Assist participants in development of a strength-based/solution-focused individualized goal and action plan that promotes self-sufficiency.
  • Assist with budgeting: Aid in developing personal budgets with participants, offering budget counseling and education.
  • Provide transportation and nutrition assistance: Arrange transportation for clients to medical appointments, and provide grocery gift cards to clients on caseload on a monthly basis. Update client files with all corresponding paperwork, including gift card receipts and proofs of appointments.
  • Insurance assistance: Help with insurance enrollments and various insurance assistance programs.
  • Provide referrals: Offer information and referral assistance to available support from social service agencies and community programs.
  • Maintain participant records: Keep accurate and complete records for all participants, adhering to Ryan White Part B Standards of Care and the Medical Case Management Manual. Create, update, and manage records in the electronic medical record, RWES, and other agency documentation systems.
  • Conduct crisis intervention and suicide risk assessment: Perform thorough suicide risk assessments and facilitate referrals to mental health services or emergency care as needed. Assist participants in developing an agreed-upon safety plan to mitigate the risk for suicide when appropriate. Follow up with participants 1-2 days after crisis intervention to assess safety and provide additional support if needed.
  • Participate in integrated care meetings: Attend weekly interdisciplinary treatment team meetings to support CHN’s holistic care model.
  • Document services provided: Maintain daily documentation of services provided to participants, notating thoroughly to ensure continuity of care. Enter Ryan White service units accurately in CAREWare.
  • Timely communication: Respond promptly to calls, emails, and other communications.
  • Complete other tasks as deemed appropriate.

JOB QUALIFICATIONS AND SKILLS:

  • Excellent communication skills, particularly listening, mediation, and writing skills.
  • Possess strong organizational skills with ability to meet a demanding workload.
  • Detail oriented to complete requirements of files and contract compliance.
  • Creative thinker/adaptive personality.
  • Fluency in the English language is required. Spanish speaking ability is an added asset but is not required.
  • Experience with computers and knowledge of Microsoft Office.
  • Sensitivity to cultural and socioeconomic characteristics of population served.
  • A commitment to empowering others to solve their own problems.
  • A conviction about the capacity of people to grow and change.
  • The ability to establish and set appropriate limits with persons served to help them gain skills and confidence.
  • The ability to work collaboratively with other personnel and/or service providers or professionals.
  • The capacity to maintain a role to empower clients and to intervene appropriately to meet service goals.

Job Type: Full-time

Pay: $23.00 per hour
