Walk-In Navigator/ Medical Case Manager

Choice Health Network is seeking a compassionate and adaptable Walk-In Navigator to provide direct support and care coordination for clients accessing our services, which include infectious disease treatment, harm reduction/syringe exchange, case management, and HIV prevention. Core responsibilities include Ryan White and ARTAS case management, linkage to medical and supportive services, and community outreach. The Walk-In Navigator reports to the Client Services Team and works closely with harm reduction, medical, and prevention staff to ensure that clients who face barriers to care receive seamless, client-centered support.

This position also involves achieving goals of the Antiretroviral Treatment and Access to Services (ARTAS) program — a strengths-based case management model to link individuals recently diagnosed with HIV to medical care.
